The Danger of Incorrect Medication Ingestion
Elderly patients often manage complex therapeutic regimens requiring them to take specific pills at different times of the day. Cognitive decline, memory issues, or poor eyesight frequently result in patients skipping doses, taking the wrong medication, or taking another dose because they forgot they had already taken it.
Traditional plastic organizers segregate pills by day but fail to prevent patients from accessing the wrong compartments or confirm if the pills were actually consumed. A secure, active reminder system is necessary.

Dynamic Compartment Control and Sensing
To eliminate errors, the Smart Pillbox restricts access to only the required dose at the scheduled time:
- Solenoid-Driven Locking: The pillbox consists of multiple circular or linear compartments. Each compartment lid is locked by a micro-solenoid or micro-servo. At the scheduled dose time, only the lid of the active compartment unlocks; all other lids remain secured.
- Consumed Sensing (IR Breakout): A miniature infrared proximity sensor or a light-dependent resistor (LDR) is placed inside each compartment. When the lid is opened and the patient tilts the box or picks up the pill cup, the sensor registers the change in light or obstacle distance. This confirms the physical removal of the medication.
- Multi-Sensory Alerting: The pillbox utilizes high-brightness flashing LEDs on the active compartment and a local buzzer playing distinct melodies to draw attention.
Remote Compliance Monitoring Loop
The device connects to the local Wi-Fi network to keep family members updated on compliance rates:
- Scheduled Release: At 8:00 AM, the morning compartment unlocks, flashing a blue LED, and the buzzer sounds.
- First-stage Alert: If the medication is not removed within 15 minutes, the buzzer sounds again at a higher volume.
- Escalation: If the sensor detects that the compartment remains full after 30 minutes, the device connects to the internet and triggers a Telegram webhook, alerting the family (e.g., "Alert: Morning medication for Mother was not taken"). This allows children to call and check immediately.
- Historical Logging: Every successful ingestion and missed alert is appended to a spreadsheet stored in the user's personal Google Drive/OneDrive, creating a compliance log for medical checkups.
Hardware Components
The Smart Pillbox leverages low-power, compact IoT components:
- Control Unit: ESP32-C3 or ESP8266 NodeMCU.
- Lid Mechanism: MG90S Micro Servos or 5V Micro Solenoid locks.
- Sensing: TCRT5000 reflective infrared sensor or GL5516 LDR.
